Evaluation and management of the thyroid nodule: role of artificial neural network
A variable diagnostic approach has been applied to the determination of optimum management strategies in the patients with nodular thyroid disease. The development and application of radionuclide scanning, sensitive ultrasonography, and needle biopsy have been helpful in distinguishing benign from malignant nodules and in selecting patients for surgery. Although ultrasonography and radionuclide  image scans are important diagnostic tools, there have little discriminate value in the distinction of benign from malignant disease. FNA is reliable test, but its reliability depends on the experience and technical skill of the clinician who perform it. For this reason, there continues to be a search for a better diagnostic procedure that yields more consistent results. CAD(computer associate diagnosis), especially ANN(artificial neural network) has been applied to variable medical fields and it is suggested their potentially successful application in radiology, in both pattern recognition and in aiding the radiologist in suggesting and ranking various diagnoses based on radiographic findings. A large number of input/output pairs provides for the training sets(100case) and testing sets(100case). !4 radiographic features of thyroid nodule (dimension, boundary, margin, multiplicity, echogenicity, cystic component, component pattern, distal enhancement, lateral shadowing, continuity and thickness of halo, calcification, invasion, metastatic lymph nodes) and 3 clinical data( sex, age and sono-probe induced tenderness) were selected as input variables, and biopsy results(benign or malignancy) as output variables to the neural network. To predict the ability of neural network, the performance of ANN and radiologist’s diagnostic accuracy were compared. In this study, ANN based on radiologic features have been expected to reduce unnecessary thyroid surgery.
